2.  1500' above lowest circling
    approach MDA.
3.  If circling not authorized, 1500' above lowest published miniumums or 2000' above airport elevation, whichever is  higher.
4.  3 SM Visibility or 2 SM more than lowest applicable visibility minimums whichever is greater.

1.   Straight-In or Circling

4.  1 SM added to landing visibility

Two Navigational Facilities
1.   Straight-In Only
2.   Precision or Non-Precision
3.   Different Suitable Runways
4.   200' to the HIGHER HAT
5.  1/2 SM to the HIGHER landing visibility of the
     two approaches.
